Change Settings to a table

Let's change the settings page to a table with 3 columns:

• Service name
• Setup
• Revoke

Right now, the UI is a little too "compressed" and hard to understand.

The Service name column will just have Gitlab or Github

The Setup column will either have the Setup button (if the service isn't configured), or a ✔️ (if the service is configured)

The Revoke column will have the Revoke button (if the service is configured). We'll change the button from ❌ to a button that says Revoke instead.

The column widths should be reasonable and provide decent spacing.
