Placeholder in custom themes fail to load #769

If the bootstrap theme files are directly copied and loaded as a custom theme the placeholder fails to load.

In the following Plunker, note two similar multiple selects, though one uses the 'cp' theme. The 'cp' theme is a direct copy of bootstrap files. Though the placeholder fails.

Also note that is it quite easy to reproduce #224 (comment) in this Plunker.

Seems like 0.11.1 is cached on cdn.rawgit.com and rawgit.com serves 0.11.2.

No longer an issue in 0.11.2
